What documents do I need for my tax appointment?

The following is a list of common items to bring or to have available for your personal tax appointment. Please note that the required documents are determined by the complexity of your return.


Required Identification (first year clients)

  • Driver's license(s) 
  • Social Security card(s) 
  • Dates of birth including dependents
  • Last year's tax return 

Possible Income Documents

  • Wage Statements (Form W-2) 
  • Retirement Income (Forms 1099-R)
  • Social Security Income (SSA-1099)
  • Unemployment Income (1099-G)
  • Information on the Sales of Stocks or Bonds 
  • Record of Purchase or Sale of Real Estate 
  • Self-Employed Business Income and Expenses (1099-Misc) 
  • Rental Income and Expenses
  • Lottery and/or Gambling Winnings or Losses   
  • State Refund Amount (1099-G)
  • Alimony Paid or Received

  • Estimated Taxes or Foreign Taxes Paid

  • Student Loan Interest (1098-E)
  • Health Insurance Marketplace Statement (Covered California 1095-A)
